Before we can complete your breast CT exam, your provider will need to send us a physician order. This ensures your scan has been approved by your provider based on your history, risk factors, and overall breast health needs.
You’ll receive an email shortly with your downloadable order form and instructions.
What You Need to Do Now
1. Fill out our patient registration form.
Please complete our patient registration form now to continue with your Breast CT appointment.
2. Schedule an appointment with your physician right away.
All Breast CT scans require an ordering provider’s referral. This is a standard step for imaging and helps ensure the scan is appropriate
based on your history, risk factors, and breast health needs.
If you already have a referring provider, please contact their office to request your imaging order. Your provider must use the Gnosis for Her requisition form linked below.

Need a Provider Quickly?
If you don’t have a provider, or can’t be seen as soon as you’d like, you’ll have the option after checkout to connect with a licensed physician through a brief telehealth medical evaluation.
These evaluations are provided by Karis Healthcare, our trusted clinical partner. Their physicians are experienced with Breast CT technology and follow evidence-based medical standards to determine whether imaging is appropriate for you.
During the telehealth visit, their physicians will review your health history, symptoms, and risk factors, answer your questions, and provide a provider order if appropriate.
If Your Provider Appointment Is After Your Scan Date
If you won’t be able to see your physician before your scheduled breast CT scan, please reschedule your appointment with us right away.
We can help you adjust your date so everything is in place for your exam.
If We Don’t Receive Your Physician Order
If a completed physician order is not received before your exam:
- Our Care Team will contact you
- We’ll help you reschedule your appointment
- Your exam will be postponed until your provider’s order is received

Cancellation & Rescheduling Policy
To ensure clarity, please review the following billing and cancellation details related to your Breast CT appointment:
A $20 reservation fee is charged at booking to hold your appointment. If you arrive for your exam, the remaining $369 will be charged on the day of your appointment for a total exam cost of $389 at our promotional rate.
- Rescheduling or canceling 72 to 24 hours before your appointment:
The $20 reservation fee will be fully refunded or transferred to your new appointment date.
- Rescheduling or canceling within 24 hours of your appointment or missed appointments:
For patients needing to reschedule, the $20 reservation fee can be transferred to your new appointment date.Patients who cancel late or miss their appointment will be charged a 50% no-show fee ($184.50).
