1. Sponsor
This sweepstakes (“Sweepstakes”) is sponsored by Gnosis For Her, in partnership with Memories4Life Management.

2. Sweepstakes Periods

• Event Sweepstakes: Begins on September 28, 2025 and ends on October 28, 2025.

• Patient Launch Sweepstakes: Begins after our first patient scheduled appointments in November and ends on December 26, 2025.

3. Eligibility
Open to legal residents of the United States who are 18 years of age or older at the time of entry.
You do not need to sign up for / purchase Gnosis For Her services to be eligible.  Employees of Gnosis For Her, Memories4Life Management, and their immediate family members are not eligible.

Void where prohibited by law.

4. How to Enter

• Event Sweepstakes: Scan the QR code at the Gnosis For Her booth or on the G4H postcard to access the entry form.

• Patient Launch Sweepstakes: Visit the G4H website or scan the QR code on any patient postcard to access the entry form.

No purchase necessary to enter or win. A Free Alternative Method of Entry (AMOE) is available by mailing a postcard with your name, email, and phone number to: Gnosis For Her Sweepstakes Entry, 300 Spectrum Center Drive, Suite 200, Irvine, CA 92618

Limit one entry per person per sweepstakes.

5. Prizes
One (1) Grand Prize Winner per Sweepstakes will receive:

• A 2-day getaway at a Memories4Life Management downtown San Clemente vacation property (blackout dates apply, including all major holidays and holiday weekends).

• A $75 gift card for dinner at a popular seaside restaurant (Downtown San Clemente).

• A $20 gift card for a quick treat at a health café (Orange County/San Clemente based).

• A 20% promotional code (G4H20) valid for use at a Memories4Life Management property (subject to standard availability and blackout dates).

6. Winner Selection and Notification
Winners will be selected via random drawing from all eligible entries received during the respective sweepstakes period. Winners will be notified via email or phone within five (5) business days of the drawing. If a winner does not respond within seven (7) days, an alternate winner may be selected.

7. Compliance with Federal and California Law
This Sweepstakes complies with:

• Federal Law: Including the Deceptive Mail Prevention and Enforcement Act (DMPE) for any mailed promotions.

• California Law:

  • California Penal Code §§ 319–329 (regulating lotteries and contests)

  • California Business and Professions Code § 17539.15 (sweepstakes regulations)

  • California Consumer Legal Remedies Act (CLRA)

To avoid classification as an illegal lottery, this sweepstakes:

• Offers a free method of entry (AMOE)

• Is based on chance, not skill

• Does not require payment or purchase to enter or win

Sweepstakes rules, odds of winning, and prize descriptions are clearly disclosed. No misleading practices are used, such as implying a participant is a winner before entry.

8. Privacy
Information collected from entrants is subject to Gnosis for Her’s Privacy Policy and will be used solely for sweepstakes administration. Personal data will not be sold or shared with third parties.

9. Limitation of Liability
Sponsor is not responsible for lost, late, incomplete, or misdirected entries or for any technical malfunctions. By entering, participants release Sponsor from any liability arising from participation or prize use.
